Change is an inevitable part of life, including at work. Knowing this, why is it that change management initiatives fail?

There are many work cultures that are set for failure when a change is required: a culture of mistrust, lack of transparency, an organizational history of failed change initiatives, absence of collaboration and engagement, and an unhealthy work environment. All of these factors contribute to a poor work culture, and in turn make change initiatives next to impossible to implement.
